L.A. Care Health Plan suffered a data breach in March 2026, when paper records were accessed or disclosed without authorization due to a physical mailing error. The incident was reported to federal regulators on April 17, 2026, and affected approximately 2,885 individuals. The compromised data included specific types of personal information, including:
- Names
- Health insurance information
- Protected health information contained in the affected paper records
Alta Orthopaedics Medical Group, Inc.
Alta Orthopaedics Medical Group, Inc. disclosed a data breach after detecting unauthorized access to its network between February 3 and February 6, 2026. The incident was discovered on March 10, 2026, and a subsequent investigation determined that patient information may have been accessed by an unauthorized actor.
The compromised data included specific types of personal information, including:
- Names
- Social Security numbers
- Dates of birth
- Driver's license or state ID numbers
- Contact information (addresses, phone numbers, and email addresses)
- Health insurance information
- Medical record and patient account numbers
- Medical diagnoses and clinical information
- Dates of service
- Provider names

- Billing codes and treatment costs
