North Central Behavioral Health Systems

North Central Behavioral Health Systems (NCBHS) recently suffered a data breach on December 2, 2025, when it detected suspicious activity inside a single employee's email account. While the investigation continues, the incident may have compromised the sensitive personal and protected health information (PHI) of its patients and employees, which can include:

  • Names
  • Addresses
  • Dates of birth
  • Social Security numbers
  • Clinical treatment information

Hennessy Advisors

Hennessy Advisors reported that it suffered a data breach around late December, when an unauthorized party accessed and copied sensitive confidential information of 12,643 victims from its network. The compromised data includes:

  • Full Names
  • Social Security numbers
  • Financial account information
  • Driver’s license numbers

Evergreen Healthcare Group

Evergreen Healthcare Group suffered a data breach on or around December 3, 2025, when an unauthorized party accessed and copied sensitive confidential information from its network, including:

  • Full names
  • Social Security numbers
  • Dates of birth
  • Medical information
