TAK Broadband LLC recently suffered a data breach between August 2 and August 12, 2025, when an unauthorized third party accessed company systems, affecting a total of 20,648 individuals. Qilin, a ransomware group, claimed responsibility for the attack, claiming to have obtained 228 GB of data, which includes:

- Driver's licenses/state identification
- Social Security numbers
- Financial Information
- Medical Information
- Health Insurance Information
Academic Urology & Urogynecology of Arizona
Academic Urology & Urogynecology of Arizona suffered a data breach between between May 18–22, 2025, when an unauthorized network intrusion compromised the personal and health data of approximately 73,281 patients, including:
- Names
- Social Security numbers
- Tax IDs
- Driver's license or government ID numbers
- Birth dates
- Financial details
- Credit/debit card info
- Health insurance data
- Medical records
Personalis, Inc.
Personalis, Inc., suffered a data breach when an unauthorized third party gained access to its networks and compromised the data of 650 individuals. The data included highly sensitive patient information, including:
- Names
- Addresses
- Dates of birth
- Social Security numbers
- Health information related to cancer genomics
- Genetic sequencing
